  • Contact our office 575-437-4533 between 8:00 AM and 5:00 PM Monday through Friday.
  • For verification, please have your insurance card available.  Also, you will be asked for your date of birth.
  • Be prepared to share the medical reason for requesting a visit to our clinic.

How to prepare and what to expect during your visit

How to schedule an appointment for an office visit

  • Please complete the medical history online if you received a username and password for our PORTALprior to office visit (required for any patient not seen within 3 years). It takes approximately 30 minutes to complete.  If you cannot complete the form online, please arrive at the clinic 1 hour in advance to complete the form on a computer at the office. 
  • You must bring your photo ID and insurance card so we can verify your insurance (without it we will regretfully have to reschedule your appointment.
  • Our provider encourages the patients to bring an updated list of medications, and a health history to each visit so we can use this information to provide more informed care.
  • If your insurance plan requires a co-pay, that will be due at the time of your visit.  We accept cash, check, VISA or MC.
  • Services not covered under insurance will be reviewed with you and require full payment before time of service.
